Home and Community Care Program

Beth Henderson joined the Health Centre in February 2005. Beth first commenced employment as the Community Health Nurse (CHN). Since that time, Beth enthusiastically and graciously accepted first the position of Home Care Nurse and now has been promoted to that of Case Manager/Coordinator. Beth’s busy schedule includes the management of the Home and Community Care Program and the Community Care Access Centre on behalf of the Beausoleil First Nation community. Beth’s experience is varied; including staff nurse at Huronia District Hospital (HDH) in the medical, surgical and obstetrics departments, 1 year at the Villa Care Centre, 2 years at the Penetang Correctional Centre, as well as experience with a naturopath, homecare and other areas. From the very beginning, Beth has felt a part of the community as well as a a valued contributor to the team at BFHC. Beth oversees personal support workers, nurses, maintenance workers and one administrative staff.
Cheryl Jamieson was originally on loan to us for a few months from the Guiding Lights Senior Centre and officially became apart of the Health Centre Team in April 2007. Chery’sl enthusiasm, energy and superior organization skills have already permeated the organization.

Cheryl brings a vast array of experience and knowledge from years of working for the Beausoleil First Nation community. She fondly recalls her first High School Summer Placement at the Day Care Centre, which really became her anchor, as she continued working in Community Services throughout the years. Her employment has included School/Community Librarian, Elementary School Secretary, Beausoleil Education Authority Secretary, Ferry Deckhand Trainee, Empowering the Youth Coordinator and finally went on to become Senior’s Activities Worker for the past five and a half years. Having worked directly with all age groups at one time or another, she has come “full circle,” as one might say. Now, employed as the Administration/Client Care Assistant for the Home & Community Care Program, she will continue to help people help themselves in our journey towards health and well-being.

Elijah Mixemong has been employed with the Home and Community Care Program for 15 years as Home Maintenance Worker. His duties consist of seasonal yard work and ensuring our clients have the necessary safety equipment installed as well as providing security checks to a number of our clients who live alone.

Elijah humbly offers his translation services on many occasions, as he is able to converse with the seniors in the community and often times acts as a liaison between clients and internal and external caregivers. Without his tireless dedication to the people, we would not be able to fulfill our responsibilities under our programming

The Beausoleil First Nation Home and Community Care Program aims to preserve and maximize the ability for the Edbendaazigig to remain as independent as long as possible at home in their own community by providing services in a holistic approach to meet an Edbendaazigig needs. The Home and Community Care Program will promote wellness to prevent or delay the need for institutional care and enhance the responsibility to the Edbendaazigig for their own health.

Integrated and coordinated services through the Beausoleil First Nation Home and Community Care Program will be accessible to all Edbendaazigig of Beausoleil First Nation in need of those services.

Our model of care management will be shared with other First Nation's communities via consultation, education, and research. We will also continue to work with individuals, communities, government and other public and private organizations to improve the lives of those individuals in need of Home and Community based care.

To encourage independence and enhance the quality of life while maintaining dignity through a holistic approach.

The Home and Community Care Program has seen recent position changes with the beginning of the new fiscal year in April 2007. However, continues to offer BFN community members the same exceptional quality of care and services.

Beth Henderson a Registered Nurse is now Coordinating the H & CC program, as well as, Case Manager. In addition to this, she attends to her daily Nursing Duties that she shares with 2 other part-time RN’s. The Administrative Client Care Assistant is Cheryl Jamieson, her primary duties are limited to the H & CC Office where she efficiently handles the office duties. The Home & Community Care Program offers members of the Beausoleil First Nation Nursing Services, Personal Care/Support (care-givers), Homemaking Services, Respite Service, Palliative Care, access to a Medical Loan Cupboard, Referrals to other Service Providers and provides Minor Home/Yard Maintenance.

This service is for infants, children, adults and seniors that have been formally assessed and determined to have a need for these type of services before being admitted to the program. Eligibility for Home & Community Care services is based on Medical Conditions, Chronic Illness, Acute Care Needs or Disabilities. Referrals may be made by clients, family members, physicians or other service providers. These services continue for as long as deemed necessary through medical assessments.
